PDHU Staff and/or Family Members
We are asking all staff who have accessed this service since January 2005 to complete this brief survey. We also welcome feedback from staff whose family members have accessed our EAP - if they have any reliable second hand feedback to contribute on behalf of that family member. Please indicate on your response whether it is being provided on your own behalf or on the behalf of a family member.
Your answers will be kept confidential and will be grouped with other responses. A summary report, which will be shared with all staff, will describe whether the service is satisfactory or if changes are planned. Thank you for your assistance.
